Investigation underway as previous $100m ‘unconscionable conduct’ fine upheld.

This incident really made me realise how important it is to follow every step of a process, especially when people’s lives could be impacted. Skipping just one step caused such a big failure, and it shows how careful we need to be in our work. It also reminded me how important backups and failover systems are — and that they need to be tested regularly, not just assumed to work. Most of all, I learned that when something goes wrong, owning up to it and being transparent is the right way to move forward.
